In the e-mail, include the following information:
Employers must accept notice of a claim from a worker and report that injury to their insurance company within five days. If the worker needs no medical treatment or is given only first aid, there is no need to notify the insurer. A record of this injury should be maintained for five years. If, however, the employer later learns that the injury has worsened and requires medical attention from a licensed practitioner, the employer must report the injury within five days by using Form 801.
All employers in Oregon that are required to provide workers' compensation coverage must display a Notice of Compliance poster in a central gathering area, such as a breakroom. Employers automatically receive a Notice of Compliance poster when they first get coverage or change coverage providers.

No payment is due for the first 3 days of disability unless the disability lasts longer than 14 days, then the first 3 days will be paid retroactively. If the worker leaves work or loses wages on the day of the injury due to the injury, that day is considered the first day of the 3 day period.(Temporary Total and Temporary Partial Disability. In-patient in hospital receives compensation from date of incapacity.)
