How to report a workplace injury

Phone: (844) 581-0828

For life-threatening medical emergencies, call 911

Preferred method: Submit the claim information online here

Another option to report a claim is via e-mail within 24 hours of the incident (or as soon as possible):

E-mail: claims@pieinsurance.com

In the e-mail, include the following information:

  • Name of business
  • Policy Number
  • Reporting party’s contact information (name, phone, e-mail)
  • The name, phone and e-mail address of injured employee
  • Date of incident
  • Location of incident (address and/or city, state)
  • Description of incident

An employer in Vermont is required by law to promptly report a work injury that results in either medical attention or one lost day of work or more.  Filing a First Report of Injury form does not make the employer liable for that injury.

What is the waiting period for lost wages in Vermont:

No payment is due for the first 3 days of disability. This is the waiting period. The 3 day waiting period includes the date of injury if the employee was not paid in full for this day.

Compensation is only payable for the first 3 days of disability (the waiting period) if the lost time lasts 7 consecutive calendar days or more. This is the retroactive period.  If exceeded, benefits are owed and payable for the entire period of disability.

The above periods apply only to total disability benefits.
