Another option to report a claim is via e-mail within 24 hours of the incident (or as soon as possible):

In the e-mail, include the following information:

  • Name of business
  • Policy Number
  • Reporting party’s contact information (name, phone, e-mail)
  • The name, phone and e-mail address of injured employee
  • Date of incident
  • Location of incident (address and/or city, state)
  • Description of incident

Employer: Complete and submit a first report of injury (form WKC-12) to notify your worker's compensation insurance company of any work injury or suspected occupational disease.  Fatal claims must be reported within 24 hours.

What is the waiting period for lost wages in the Wisconsin:

No payment is due for the first 3 days of disability unless the disability lasts for more than 7 calendar days. The first 3 days are the waiting period.  Benefit eligibility begins on day 4 and runs forward. If the period of disability exceeds 7 calendar days, the first 3 days are payable. This 7 day period is the retroactive period.
